legislation is implemented that effects retiree's medical <br /> insurance. <br /> The City will contact the health care providers and seek <br /> the opportunity to place retirees under the active plans. <br /> 16.3 Dental <br /> Effective on the first day following City Council approval <br /> of this agreement, the City shall contribute an amount not <br /> to exceed sixty -five dollars and eighty -five ($65.85) per <br /> month per employee to provide dental care benefits under <br /> the City's present dental plan for the individual employee <br /> and eligible dependents. Effective January 1, 1996, said <br /> maximum monthly contribution will be increased to an <br /> amount not to exceed seventy -two ($72.00) dollars per <br /> month. Effective January 1, 1997, said maximum monthly <br /> contribution shall be increased to an amount not to exceed <br /> eighty -one ($81.00) dollars per month. In addition, <br /> following City Council adoption of this Memorandum of <br /> Understanding, the City will increase the maximum <br /> orthodontia coverage to $1,500. <br /> 16.4 Life, Accidental Death and Dismemberment Insurance <br /> As soon as practical but no later than October 1, 1990, <br /> the City shall increase the present employee $10,000 life <br /> and $10,000 accidental death and dismemberment insurance <br /> coverage to $25,000 life and $25,000 accidental death and <br /> dismemberment insurance coverage. The monthly premium for <br /> life insurance coverage shall be paid by the City. <br /> Employee may also participate in the City's supplemental <br /> life insurance program at their expense. <br /> 16.5 Lona -Term Disability <br /> Effective July 1, 1988 the City's provision of long term <br /> disability insurance coverage to employees under the <br /> Canada Life Plan shall terminate and the City shall have <br /> no further responsibility to provide long term disability <br /> benefits except as provided below. <br /> Effective September 1, 1990, the City shall pay to the <br /> Union the sum of fifteen dollars ($15.00) as the monthly <br /> premium of each employee enrolled in the union's long term <br /> disability plan. Said payments shall be made in arrears <br /> monthly. Should the monthly premium of the Union's long <br /> term disability plan drop below fifteen dollars per month, <br /> the dollar difference shall be added one time to the rate <br /> of pay set forth in Section 5.1 of this agreement. <br /> - 20 - <br />
