Maximum City Contribution Amounts <br />Monthly Amount Covered Classification <br />$379 Firefighter <br />$379 Fire Engineer <br />$379 Fire Captain <br />$561 Fire Inspector <br />$561 Hazardous Materials Coordinator <br />Spousal Coverage <br />An eligible employee may present proof of medical and /or dental insurance coverage through their <br />spouse's employment, and request cancellation of the City insurance coverage. Upon request and <br />cancellation of the employee's coverage, the City will pay an amount into a City deferred <br />compensation plan in the employee's name. The amount the employee is eligible for will be based <br />on the Operating Engineer's family or married premium rate that the employee qualifies for at the <br />time the insurance is canceled. The City will take the amount the employee is eligible for, minus <br />any required premium for continuing insurance, and then pay 80% of that amount into a deferred <br />compensation plan. The total amount paid by the City will not exceed the maximum City benefit <br />allowable for medical /dental insurance coverage. <br />The amount to be paid into the deferred compensation plan will be adjusted annually based on the <br />change in the Operating Engineers premium rates, and is not to exceed the maximum City benefit <br />allowable for medical /dental insurance coverage <br />Any employee requesting this will be required to meet all rules and conditions covering the <br />administration of the deferred compensation plan, and the health and dental plan. <br />C. LONG TERM DISABILITY PLAN <br />Effective July 1, 1985, eligible employees shall be provided with a Long Term Disability Benefits <br />Program as selected, administered or revised by the City. Effective for the payment of premiums <br />required for October 1988 coverage, the City shall contribute an amount not to exceed Twenty <br />Two Dollars and Forty Six Cents ($22.46) per month per eligible employee toward the monthly <br />premium cost; provided however, that in no event shall the City be obligated to contribute an <br />amount in excess of Twenty Two Dollars and Forty Six Cents ($22.46) toward the employee's <br />premium cost or the actual cost if a lesser amount. <br />D. LIFE INSURANCE <br />The City shall provide Ten Thousand ($10,000) of term life insurance coverage per eligible <br />employee, which includes an equivalent of accidental death and dismemberment coverage. <br />25 <br />
