Box 944231 Sacramento, CA 94244-2310 EPN REQUESTER CODE After enrolling, employers will be assigned an identifying requester code. The requester code will be shown on the duplicate copy of the Pull Notice Contract. This number must be shoWn on all future enrollment or deletion requests and correspo~ denco submitted to the departmere to avoid processing delays. PLEASE NOTE: The requester code is a confidential number and will not be faxed or released over the telephone. Employers should not provide the requester code to unauthorized persons. ACCOUNT CHANGES The enrollment forms (INF 1100, and INF 1102) can be used to report address, contact person and telephone number changes to the EPN account. Changes may also be submitted in writing on company letterhead. Address changes not toperted promptly may delay the receipt of the driver's DL records. CLOSING ACCOUNTS Employers closing their account and removing their company from the EPN program must notify the department in wriling, Any outstanding balance on the ABIS account is due and must be paid in full. NOW TO REQUEST FORMS A token supply of forms (5 or less) may be obtained by wdting the EPN Unit or by calling (916) 657-6346. Forms may be photocopied. PLEASE NOTE: Requests for information on driver's records received by telephone can only be verified. No new information will be given (e.g., reinstatements, etc.)
