The requester code will be shown on the duplicate <br /> copy of the Pull Notice Contract. This number must be shoWn on all future enrollment or deletion requests and correspo~ <br /> denco submitted to the departmere to avoid processing delays. <br /> <br /> PLEASE NOTE: The requester code is a confidential number and will not be faxed or released over the telephone. <br /> Employers should not provide the requester code to unauthorized persons. <br /> <br /> ACCOUNT CHANGES <br /> <br /> The enrollment forms (INF 1100, and INF 1102) can be used to report address, contact person and telephone number <br /> changes to the EPN account. Changes may also be submitted in writing on company letterhead. Address changes not <br /> toperted promptly may delay the receipt of the driver's DL records. <br /> <br />CLOSING ACCOUNTS <br /> <br />Employers closing their account and removing their company from the EPN program must notify the department in wriling, <br />Any outstanding balance on the ABIS account is due and must be paid in full. <br /> <br />NOW TO REQUEST FORMS <br /> <br />A token supply of forms (5 or less) may be obtained by wdting the EPN Unit or by calling (916) 657-6346.
