EXHIBIT B <br />CONDITIONS OF APPROVAL <br />PCU1 -141, Cathy Hirata and Sandy Gablenz, Where Music Begins <br />5460 Suuo~ Boulevard, Suites 3 and 4 <br />December 14, 2005 <br />l . The location and operation of the proposed use shall conform substantially to <br />Exhihit A (the site plan, floor plans, and details of operation), dated "Received, <br />November 1 8, 2005," on File with the Ylanning Department, except as modified by <br />the following conditions of approval- Minor changes to the approved operation and <br />schedule may be approved by the Planning Director if found to be in substantial <br />compliance with the proposal. <br />2. Additional hours and acti vitics beyond what was stated in the applicant's statement of <br />operation, dated "Received, November 1 8, 2005," on file in the Planning Dcparttnent <br />tray be approved by the Planning Director if found. to be in substantial conformance <br />with he approval- The Planning Director may also refer the matter to the Planning <br />Coniniission if the proposed changes would be significant. <br />3. If operation of this use permit results in conflicts pertaining to parking, noise, <br />tralt3c/circulation, or other factors, at the discretion of the Pla.iming Director, this <br />conditional use permit may be submitted to the Planning Con~nzission for its <br />subsequent review at a public hearing. If necessary, the Planning Commission may <br />modify or add conditions of approval to mitigate such impacts or may revoke the said <br />conditional use permit approval. <br />4. Prior to the opening of the music studio, the applicant shall soundproof the tenant <br />space. The tenant improvement plan shall detail the soundproofing to the common <br />wall share by the adjoining tenants- A building permit, if required., shall be obtained <br />prior to commencement of any work- <br />5. Prior to the opening of the music studio, the applicant shall contact t]ie Pire Marsha] <br />to conform to Fire Code requirements, if applicable. <br />6. No changes to the exterior of the building shall be made without prior approval firo~r~ <br />the Planning Department. <br />7. Any signage shall conform to Oak Hills Shopping Center sign program. <br />8. The applicant shall obtain a City business license prior to the opening of the music <br />studio. <br />{end) <br />
