Box 520 Pleasanton, CA 94566 (Reference Bid #OSC-05-03-ST) Mailed or hand-delivered proposals must be received by 2:00pm on Monday, November 28,2005. Proposals received after the specified time will not be considered. Other Reauirements Contract crews shall be identified at all times either by a Company uniform or by a safety vest worn outside other clothing with the Company name on the back. Vehicles will have a Company sign on both front doors. Contractor shall obtain at his own expense and place whatever signs, lights, barricades, or other safety devices as necessary to prevent accidents, injuries, or damages, consistent with the City, County and State Work Area Traffic Control Handbook. The selected firm will be required to execute the City's standard form contract agreement upon successfully negotiating a fmal scope and fee proposal with the City. A copy of said agreement is available for review. '" The selected firm shall be required to submit certificates of worker's compensation insurance and liability insurance in accordance with the requirements of the standard contract agreement. The selected firm shall each be required to obtain a City business license. Selection Procedures Selection of contractor will be deemed as the most qualified in terms of past experience and statement of references, as well as offering the most competitive rates.
