Resolution No. PC-94-25 Page 4 EXHIBIT B Resolution No. PC-94-25 Conditions of Approval Case UP-94-23 1. The proposed use shall conform substantially to the project description, written narrative, site plan, and floor plan, Exhibit "A", dated "Received May 4, 1994" on file with the Planning Department, except as modified by these conditions. 2. If the operation of the use results in major conflicts pertaining to parking, noise, traffic, or other impacts, at the discretion of the Planning Director, this conditional use permit may be referred to the Planning commission for subsequent review at a public hearing. If necessary, the Commission may modify or add conditions of approval to mitigate such impacts, or may revoke the said conditional use permit. 3. If additional activities beyond that proposed in the written narrative, dated "Received May 4, 1994" on file with the Planning Department are desired, prior city review and approval is required. Such modifications shall be submitted to the Planning Director, who shall refer the matter to the Planning Commission if judged to be substantial. 4. only two licensed massage therapists shall be allowed with this permit. 5. The hours of operation of the therapeutic massage studio shall not exceed Monday through Saturday from 8:00 a.m. to 8:00 p.m. and closed on Sundays. 6. The applicant shall obtain all building and other applicable City permits for the use and structure prior to the commencement of the use. 7. This use permit is not transferrable to another address. A new application must be made if the applicant relocates to another site. 8. No signage for the business is approved as part of this application. Any signage shall conform to the previously- approved sign program for the building and shall be subject to the review and approval of the planning Director prior to installation. {end} 1I [l
