<br />Resolution No. PC-94-25 <br />Page 4 <br /> <br />EXHIBIT B <br />Resolution No. PC-94-25 <br />Conditions of Approval <br />Case UP-94-23 <br /> <br />1. The proposed use shall conform substantially to the project <br />description, written narrative, site plan, and floor plan, <br />Exhibit "A", dated "Received May 4, 1994" on file with the <br />Planning Department, except as modified by these conditions. <br /> <br />2. If the operation of the use results in major conflicts <br />pertaining to parking, noise, traffic, or other impacts, at <br />the discretion of the Planning Director, this conditional use <br />permit may be referred to the Planning commission for <br />subsequent review at a public hearing. If necessary, the <br />Commission may modify or add conditions of approval to <br />mitigate such impacts, or may revoke the said conditional use <br />permit. <br /> <br />3. If additional activities beyond that proposed in the written <br />narrative, dated "Received May 4, 1994" on file with the <br />Planning Department are desired, prior city review and <br />approval is required. Such modifications shall be submitted <br />to the Planning Director, who shall refer the matter to the <br />Planning Commission if judged to be substantial. <br /> <br />4. only two licensed massage therapists shall be allowed with <br />this permit. <br /> <br />5. The hours of operation of the therapeutic massage studio shall <br />not exceed Monday through Saturday from 8:00 a.m. to 8:00 p.m. <br />and closed on Sundays. <br /> <br />6. The applicant shall obtain all building and other applicable <br />City permits for the use and structure prior to the <br />commencement of the use. <br /> <br />7. This use permit is not transferrable to another address. A new <br />application must be made if the applicant relocates to another <br />site. <br /> <br />8. No signage for the business is approved as part of this <br />application. Any signage shall conform to the previously- <br />approved sign program for the building and shall be subject to <br />the review and approval of the planning Director prior to <br />installation. <br /> <br />{end} <br /> <br />1I <br /> <br />[l <br />
