<br />18.114.050 <br /> <br />zoning Districts - Permitted <br /> <br />Any Ad~lt Entertainment Establishment shall be permitted only <br />in the commercial zones listed in Table 18.44.090 of Chapter <br />18.44. <br /> <br />18.114.060 <br /> <br />Adult Entertainment Establishment Permit <br />Required <br /> <br />A. Prior to commencing any work pertaining to the <br />development, construction, reconstruction, relocation, <br />conversion, alteration, expansion, or establishment of <br />any Adult Entertainment Establishment, the applicant <br />shall submit to the City Planning Department an <br />application for an Adult Entertainment Establishment <br />permit on a form approved by the Planning Department and <br />a site plan. A fee therefor shall be paid pursuant to <br />section 18.114.070 of this Chapter. The application and <br />site plan are required for purposes of verification that <br />the request complies with the design and performance <br />standards, and is in conformity with the locational <br />criteria, set forth in this Chapter. <br /> <br />B. The applicant shall also submit to the Police Chief an <br />application for an Adult Entertainment Establishment <br />Permit. Said application shall be in writing on a form <br />prescribed by the Police Chief and shall be signed by the <br />applicant. The application shall set forth the exact <br />nature of the activities proposed to be conducted, the <br />proposed place of businesses and facilities therefor, and <br />the name and address of each applicant. The Chief of <br />Police may require the applicant to allow fingerprints to <br />be taken for the purpose of establishing identification. <br />Any applicant shall furnish the following information: <br /> <br />1. The previous addresses of each applicant, if any, <br />for a period of three years immediately prior to <br />the date of the application and the dates of <br />residence of each. <br /> <br />2. written proof that the applicant is at least <br />eighteen years of age. <br /> <br />3. The applicant shall allow the Police Department <br />official who processes the application to take <br />photographs of the applicant. <br /> <br />4. Applicant's height, weight, color of eyes and hair. <br /> <br />5. Business, occupation or employment history of the <br />applicant for the three years immediately preceding <br />the date of the application. <br /> <br />8 <br />
