<br />. . <br /> <br />Resolution No. PC-95-81 <br />Page 4 <br /> <br />EXHIBIT "B" <br />Conditions of Approval <br />Case UP-95-62 <br /> <br />1. The proposed use shall conform substantially to the project <br />description, written narrative, site plan, and floor plan, Exhibit <br />"A," dated "Received September 29, 1995," on file with the Planning <br />Department, except as modified by these conditions. <br /> <br />2. If the operation of the use results in major conflicts pertaining <br />to parking, noise, traffic, or other impacts, at the discretion of <br />the Planning Director, this conditional use permit may be referred <br />to the Planning Commission for subsequent review at a public <br />hearing. If necessary, the Commission may modify or add conditions <br />of approval to mitigate such impacts, or may revoke the said <br />conditional use permit. <br /> <br />3. If additional activities beyond that proposed in the written <br />narrative, dated "Received September 29, 1995," on file with the <br />Planning Department are desired, prior City review and approval are <br />required. Such modifications shall be submitted to the Planning <br />Director, who shall refer the matter to the Planning Commission if <br />determined to be substantial. <br /> <br />4. Only one licensed massage therapist shall be allowed to operate <br />under the subject use permit. <br /> <br />S. The hours of operation of the therapeutic massage office shall be <br />Monday through Friday from 9:00 a.m. to 8:00 p.m. and one Saturday <br />a month from 11:00 a.m. to 3:00 p.m. <br /> <br />6. The applicant shall obtain all building and other applicable City <br />permits for the use and structure prior to the commencement of the <br />use. <br /> <br />7. This use permit is not transferable to another address.
