<br />Resolution No. PC-9S-80 <br />Page 4 <br /> <br />EXHIBIT "B" <br />Conditions of Approval <br />Case UP-9S-63 <br /> <br />1. The proposed use shall conform substantially to the project <br />description, written narrative, site plan, and floor plan, <br />Exhibit "A," dated "Received September 29, 1995," on file with <br />the Planning Department, except as modified by these <br />conditions. <br /> <br />2. If the operation of the use results in major conflicts <br />pertaining to parking, noise, traffic, or other impacts, at <br />the discretion of the Planning Director, this conditional use <br />permit may be referred to the Planning Commission for <br />subsequent review at a public hearing. If necessary, the <br />Commission may modify or add conditions of approval to <br />mitigate such impacts, or may revoke the said conditional use <br />permit. <br /> <br />3. If additional activities beyond that proposed in the written <br />narrative, dated "Received September 29, 1995," on file with <br />the Planning Department are desired, prior City review and <br />approval are required. Such modifications shall be submitted <br />to the Planning Director, who shall refer the matter to the <br />Planning Commission if determined to be substantial. <br /> <br />4. Only three licensed massage therapists shall be allowed to <br />operate under the subject use permit. <br /> <br />S. The hours of operation of the therapeutic massage facility <br />shall be Monday through Saturday from 8:00 a.m. to 8:00 p.m. <br /> <br />6. <br /> <br />The applicant shall <br />City permits for <br />commencement of the <br /> <br />obtain all building and other applicable <br />the use and structure prior to the <br />use. <br /> <br />7. This use permit is not transferable to another address. A new <br />application must be made if the applicant relocates to another <br />site. <br /> <br />8. No signage for the business is approved as part of this <br />application. Any signage shall conform to the previously- <br />approved sign program for the building and shall be subject to <br />the review and approval of the Planning Director prior to <br />installation. <br /> <br />9. No massage therapist may operate until the required Police <br />Department approvals and registrations are obtained. Such <br />registrations shall remain in effect as long as the business <br />is in operation. <br />{end} <br /> <br />- <br />
