Exhibit B Conditions of Approval PCUP-1l3, Virginia RossiILongshore Tutoring Center 1020 Serpentine Lane, Suite 105 August 11, 2004 I. The location and operation of the proposed use shall conform substantially to Exhibit "A" (the site plan, floor plans, and details of operation), dated "Received, July 2, 2004", on file with the Plarming Department, except as modified by the following conditions of approval. Minor changes to the approved operation and schedule may be approved by the Planning Director if found to be in substantial compliance with the proposal. 2. Additional hours and activities beyond what was stated in the applicant's statement of operation, dated "Received July 2, 2004", on file in the Plarming Department may be approved by the Plarming Director if found to be in substantial conformance with he approval. The Plarming Director may also refer the matter to the Plarming Commission if the proposed changes would be significant. 3. If operation of this use permit results in conflicts pertaining to parking, noise, traffic/circulation, or other factors, at the discretion of the Planning Director, this conditional use permit may be submitted to the Plarming Commission for their subsequent review at a public hearing. Ifnecessary, the Plarming Commission may modify or add conditions of approval to mitigate such impacts, or may revoke the said conditional use permit approval. 4. Prior to operation, the applicant shall contact the Building Inspection Department and the Fire Marshal to conform to Building Code and Fire Code requirements, if applicable. The applicant shall obtain a building permit prior to commencement of any work, if required. No changes to the exterior of the building shall be made without prior approval from the Plarming Department. 5. The applicant shall obtain a City business license as soon as this use permit approval becomes effective. {end} Ifl -
