The proposed use shall conform substantially to the written narrative, licensing <br />information, site plan, and floor plan, "Exhibit A," dated "Received June 18, 1996," on <br />file with the Planning Department, except as modified by these conditions. <br /> <br />2. If the operation of the use results in major conflicts pertaining to parking, noise, traffic, or <br />other impacts, at the discretion of the Planning Director, this conditional use permit may <br />be referred to the Planning Commission for subsequent review at a public hearing. If <br />necessary, the Commission may modify or add conditions of approval to mitigate such <br />impacts, or may revoke the said conditional use permit. <br /> <br />3. If additional activities beyond that proposed in the written narrative, dated "Received <br />June 18, 1996," on file with the Planning Department are desired, prior City review and <br />approval are required. Such modifications shall be submitted to the Planning Director, <br />who shall refer the matter to the Planning Commission if determined to be substantial. <br /> <br />4. No more than two licensed massage therapists shall operate in the designated tenant suite <br />at one time. <br /> <br />5. The hours of operation for Bodylines Massage & Fitness shall be limited to Monday <br />through Saturday from 9:00 a.m. to 8:30 p.m. Minor changes to the hours of operation <br />may be submitted for the review and approval by the Planning Director. <br /> <br />6. The applicant shall obtain all building and other applicable City permits for the use and <br />structure prior to the commencement of the use. <br /> <br />7. This use permit is not transferable to another address. A new application must be made if <br />the applicant relocates to another site. <br /> <br />8. No signage for the business is approved as part of this application. Any signage shall <br />conform to the previously-approved sign program for the building and shall be subject to <br />the review and approval of the Planning Director prior to installation. <br /> <br />9. No massage therapist may operate until the required Police Department approvals and <br />registrations are obtained. Such registrations shall remain in effect as long as the <br />business is in operation. <br /> <br />10. A business license shall be obtained from the City of Pleasanton for each of the <br />individual businesses upon approval of said use permit. <br /> <br />{end} <br />
