Page 7 EXHIBIT A CITY OF PLEASANTON HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM BETWEEN CITY OF PLEASANTON AND Spectrum Community Services JULY 1, 2025 THROUGH JUNE 30, 2026 Project No. 2026102 Meals on Wheels The project to be undertaken by Spectrum Community Services shall involve: Spectrum’s Meals on Wheels Program provides nutritious meals and vital safety checks daily to disabled/homebound Pleasanton seniors/disabled adults. We will deliver 19,500 or more meals to 125 seniors. The project shall conform substantially to the scope of work described above. [However, the targets and service goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City of Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows: a) If the proposed change is in substantial conformance with the original project scope as determined by the City’s HHSG program administrator, the change may be approved by City staff. b) If the proposed change is in partial conformance with the original project scope as determined by the City’s HHSG program administrator, the change will be submitted for review and approval by the City’s Human Services Commission and/or Housing Commission. c) If the proposed change is not in conformance with the original project scope as determined by the City’s HHSG program administrator, the change will be submitted for review and approval by the City’s Human Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a change to the Annual Action Plan.
