Page 7 <br />EXHIBIT A <br /> <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br /> <br />CITY OF PLEASANTON <br />AND <br />Community Resources for Independent Living (CRIL) <br /> <br />JULY 1, 2025 THROUGH JUNE 30, 2026 <br /> <br />Project No. 2026093 <br />Housing and Independent Living Skills <br /> <br />The project to be undertaken by Community Resources for Independent Living (CRIL) shall involve: <br /> <br />CRIL's Housing & Independent Living Services will provide Pleasanton residents who have a disability with housing search <br />assistance, peer counseling, independent living skills & travel training. In this program, CRIL provides an array of services to <br />encourage independence - this is accomplished through peer work. 84% of CRIL's staff have a disability. In FY 2025/26, 30 <br />Pleasanton will receive direct services, and 75 Pleasanton residents will receive information and referral services. <br /> <br />The project shall conform substantially to the scope of work described above. [However, the targets and service <br />goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City <br />of Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows: <br /> <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City’s HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission. <br />c) If the proposed change is not in conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br />Docusign Envelope ID: 147D45DF-BAAB-42E4-A07D-34B73402BC8DDocusign Envelope ID: 05EAC57C-BC9F-470F-9E35-D5EDE3C89694
