No new unduplicated Pleasanton clients were served between January and June 2025, as <br />all clients were housed and served during the first half of the fiscal year. <br />Two households exited the program in the beginning of the reporting period, three households are currently housed and one remaining household <br />in housing search. <br />In total, Abode served six households. These included one household with two people, two households with five people each, and one single-person <br />households. <br />13. Describe how your project/program is addressing the critical human services needs in the "City of Pleasanton Human Services <br />Strategic Plan Needs Assessment Update 2018-2022". <br />Report 1 <br />N/A <br />Report 2 <br />Abode collaborates with more than 30 service providers in order to help program participants access a wide breadth of services to meet their <br />needs. Collaborators include Tri-Valley organizations that are embedded in the local community and highly accessible to participants. <br />14. Describe how your project/program is addressing the "Strategic Goals" and "Priority Needs" identified in the "City of Pleasanton U.S. <br />Department of Housing and Urban Development (HUD) FY 2022-2024 Consolidated Plan". <br />Report 1 <br />N/A <br />Report 2 <br />The TriValley Rapid Rehousing Program provides homeless families in the Pleasanton area with housing placement and a gradually decreasing <br />rental subsidy to help families stabilize and become self sufficient. We also provide case management to work on accomplishing housing stability <br />goals developed together with the family. Households will work to increase their income so they can afford their full rent once they exit the <br />program. The program will help with some homeless prevention which includes back rent and motels to keep families from becoming homeless. <br />15. Describe the original purpose for this grant. Will you spend your entire grant? If not, explain why your agency will not be able to <br />spend the entire grant amount. <br />Report 1 <br />The original purpose of this grant is to provide services to six individual households. We are on track to spend the entire grant by end of fiscal year. <br />Report 2 <br />The original purpose is to provide service/resources and house homeless individuals. The grant also allows us to provided prevention services if <br />needed. <br />
