Exhibit A <br />Scope of Services <br />CityServe of the Tri-Valley <br /> <br />CityServe of the Tri-Valley will deliver comprehensive, compassionate, and accessible <br />support to Pleasanton residents through case management. This will include: <br />• Onsite Support: <br />o Case managers (Care Coordinators) will be available daily during designated <br />drop -in hours at the Pleasanton Senior Center to address direct referrals. <br />• Care Coordination: <br />o Each client will complete an intake assessment to create a personalized <br />Care Plan using CityServe’s Care Wheel model (housing, health, nutrition, <br />transportation, emotional wellness.) <br />• Follow Up: <br />o Regular, weekly, monthly or as needed check-ins, with care plan updates. <br /> <br />Docusign Envelope ID: 2E5FFDB8-9C04-41BA-B315-94981CF3ADBC
