<br />Resolution No. PC-97-45 <br />Page 3 <br /> <br />EXHIBIT "B" <br />Conditions of Approval <br />Case UP-97-32 <br />Frances L. Blasque <br /> <br />1. The proposed use shall conform substantially to the written narrative, site plan, and floor plan, <br />Exhibit "A", dated "May 1997", on file with the Planning Department, except as modified by <br />these conditions. <br /> <br />2. If operation of the proposed use results in conflicts pertaining to parking, noise, traffic, or <br />other impacts, at the discretion of the Planning Director, this use permit may be submitted to <br />the Planning Commission for subsequent review at a public hearing. If necessary, the <br />Commission may modify or add conditions of approval to mitigate such impact, or may revoke <br />the use permit. <br /> <br />3. If additional activities beyond that proposed in the written narrative, dated "May 1997", on file <br />with the Planning Department are desired, prior City review and approval are required. Such <br />modifications shall be submitted to the Planning Director, who shall refer the matter to the <br />Planning Commission if determined to be substantial. <br /> <br />4. No more than one licensed massage therapist shall operate within the designated tenant suite. <br /> <br />5. The hours of operation for the massage therapist shall be limited to Monday through Friday, 8 <br />a.m. to 7 p.m. and Saturday, 8 a.m. to 3 p.m.. Minor changes to the hours of operation may be <br />submitted for review and approval by the Planning Director. <br /> <br />6. The applicant shall obtain all other applicable City permits prior the commencement of the use. <br /> <br />7. This use permit is not transferable to another address. A new application must be made if a <br />massage therapist relocates to another site. <br /> <br />8. No signage for the business is approved as part of this application. Any signage shall conform <br />to the previously-approved sign program for the building and shall be subject to the review and <br />approval of the Planning Director prior to installation. <br /> <br />9. No massage therapist may operate until the required Police Department approvals and <br />registrations are obtained. Such registrations shall remain in effect as long as the business is in <br />operation. <br /> <br />{end} <br />
