HCL 25-1064 H327-01 <br />Issued to: Axis Community Health <br />Effective Date: 07/01/2025 at 12:01 a.m. Expiration Date: 07/01/2026 at 12:01 a.m. Additional Contribution: Per Contract <br />It is understood and agreed that coverage afforded by Section 3 (Bodily Injury and Property Damage Liability) <br />and Section 4 (Personal Injury, Advertising Injury and Discrimination Liability) of this Contract is extended to <br />any person or organization for whom the Named Member or Subsidiary is required by a written agreement to <br />obtain and maintain insurance or other coverage as a Supplemental Member pursuant to Section 7.2, but only for <br />legal liability arising out of the acts, errors or omissions of the Named Member or a Subsidiary solely in the <br />performance of the written agreement between the Named Member or Subsidiary and the Supplemental Member. <br />This Amendment does not extend coverage for the acts, errors or omissions of third parties or their employees. <br />This Amendment extends protection to the Supplemental Member prior to any applicable insurance or other <br />coverage or self-insurance. <br />ALL OTHER TERMS, CONDITIONS AND EXCLUSIONS REMAIN UNCHANGED. <br />4� OAF t <br />Authorized Representative of BETARMA <br />
