EXHIBIT A – FY 24/25 <br />record and store short video movies of the sewer/storm defects and include verbal audio when <br />necessary. The operator(s) shall be Pipeline Assessment Certification Program <br />(PACP/MACP) certified. NASSCO PACP/MACP format shall be used to assess the pipeline <br />conditions. The inspections shall document the following conditions and any other pertinent <br />observations: <br />a. Cracks - radial and longitudinal, hairline or open. <br />b. Laterals (identify active or inactive, if possible) - quadrant location, protruding, cracks, <br />roots. <br />c. Corrosion - iron stains, roughened aggregate, exposed aggregate, exposed rebar, or exposed <br />soil. <br />d. Offsets/sags - 1-inch, 2-inch, 4-inch, etc. <br />e. Roots - light, moderate, heavy, or matted roots. <br />f. Grease - light, moderate, or heavy. <br />g. Infiltration - water stains, dripping water, or stream of water. <br />h. Other - holes in pipe, missing tile. <br />16. PACP/MACP pipe ratings shall be included for each pipe segment inspected. All videos <br />shall be of good quality MPEG-4 format and shall clearly show the above stated conditions. The <br />date, identification of sewer/storm section by upstream and downstream manhole locations, and <br />footage shall be displayed on the video data view at all times. <br /> <br />17. MPEG-4 videos, pictures and databases must be submitted on external portable external <br />hard drive. If a video and/or the capture images in the database do not clearly show the pipe <br />conditions due to an inadequate amount of light or are of such poor quality that the City is unable <br />to evaluate the condition of the sewer or locate sewer service connections, the CONTRACTOR <br />shall re-televise that section of the pipe at CONTRACTOR'S expense. <br /> <br />DELIVERABLES: <br /> <br />1. Deliverables shall include CCTV inspection videos, pictures and databases in an external <br />hard drive as mentioned herein; manhole inspection reports; printed CCTV inspection reports; <br />summary of CCTV inspection reports in excel format (see details below),; MPEG-4 format color <br />video. <br />2. Submit summary of the CCTV inspection reports in excel format showing the following: <br />inspection ID, pipe segment reference, date of inspection, street name, upstream manhole no. <br />designation, downstream manhole no. designation, sewer/storm use, direction of flow, flow <br />control, size of pipe, type of pipe, shape of pipe, pipe joint length, total pipe length, actual pipe <br />length surveyed, pre-cleaning method use, weather condition and the overall pipe rating condition <br />of the pipe segments (from MH to MH) indicating the following ratings 1 - if the pipe is in <br />excellent condition, 2 if the pipe is in good condition, 3 - if the pipe is in fair condition, 4 if the <br />pipe is in poor condition and 5 - if the pipe is in bad condition and needing immediate removal and <br />replacement. <br />RATE SHEET <br />The cost quoted below shall include all taxes and all other charges, including travel expenses, <br />and is the cost the City will pay for the term of any contract that is a result of this bid. <br /> <br />Docusign Envelope ID: D32E8023-554D-4B88-88F3-521E56BCE792
