Docusign Envelope ID : ADE0408F-CC99-4CDB-A878-33580FFB2F7D <br />EXHIBIT A <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br />CITY OF PLEASANTON <br />AND <br />Habitat for Humanity East Bay/Silicon Valley, Inc. <br />JULY 1, 2024 THROUGH JUNE 30, 2025 <br />Project No. 2024596 <br />Housing Rehabilitation Program <br />The project to be undertaken by Habitat for Humanity East Bay/Silicon Valley, Inc . shall involve: <br />Administration of the City 's mobile home and single-family housing rehabilitation loan and grant program which targets <br />low-income homeowners at or below 80% of Area Median Income . In FY 2024125, Habitat for Humanity 's target is to <br />complete four (4) housing rehabilitation grant projects . <br />Through the Pleasanton Housing Rehabilitation Program ("Program "), Habitat partners with homeowners and provides critical <br />home repairs . Participants of this program are cost-burdened and economically vulnerable to foreclosure and do not have the <br />resources to invest in needed home improvements. Their once high-quality houses have deteriorated, lost value, and are <br />becoming uninhabitable . By empowering residents of Pleasanton through critical health and safety repairs , we will meet the <br />need for healthy, safe and affordable living conditions . Repairs enable households to experience greater safety, health, comfort, <br />and financial stability by decreasing their likelihood of foreclosure and allow them to safely age in place. <br />The project shall conform substantially to the scope of work described above . [However, the targets and service <br />goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City <br />of Pleasanton's HHSG administrator in the form of a letter. Requests shall be processed as follows: <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City's HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the original project scope as determined by the City 's <br />HHSG program administrator, the change will be submitted for review and approval by the City's Human <br />Services Commission and/or Housing Commission. <br />c) If the proposed change is not in conformance with the original project scope as determined by the City's <br />HHSG program administrator, the change will be submitted for review and approval by the City's Human <br />Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br />Page 7 <br />Docusign Envelope ID: 813DC707-E5B0-41F2-A51F-8A2E6CE38AEF
