EXHIBIT A <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br />CITY OF PLEASANTON <br />AND <br />Tri-Valley REACH, Inc. <br />JULY 1, 2024 THROUGH JUNE 30, 2025 <br />Project No. 2024619 <br />Home Improvement Projects, Repair & Maintenance <br />The project to be undertaken by Tri-Valley REACH, Inc., shall involve : <br />REACH is seeking funding specifically for identified projects or major repairs , home improvements or <br />rehabilitation projects for the Pleasanton properties. These projects are identifed in Attachment 8-1 based <br />upon current projected needs but are not limited to those projects should another critial project become <br />necessary and take priority. REA CH currently supports 16 Pleasanton tenants and upon completion of the <br />Hansen ADU, which is projected to be August 2024, REACH will have 7 homes supporting 18 tenants . <br />On an annual basis REACH must raise funds for routine maintenance/repair expenses and additional <br />identified projects . REACH tenants are classified as extremely low income, at or below 30% BM/ and on <br />average their rent ranges between $350 -$500 per month. REACH provides "affordable" housing for this <br />underserved population and the tenant's rent does not generate enough income to cover all annual expenses . <br />Should REACH not receive funding , repairs and maintenance will take priority and any identified projects will <br />be postponed until REACH has 100% funding. <br />The project shall conform substantially to the scope of work described above . [However, the targets and service <br />goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City <br />of Pleasanton's HHSG administrator in the form of a letter. Requests shall be processed as follows : <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City's HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the original project scope as determined by the City's <br />HHSG program administrator, the change will be submitted for review and approval by the City's Human <br />Services Commission and/or Housing Commission. <br />c) If the proposed change is not in conformance with the original project scope as determined by the City's <br />HHSG program administrator, the change will be submitted for review and approval by the City's Human <br />Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br />Page 7 <br />Docusign Envelope ID: 11C641AB-2891-4975-A4A8-8C9C82562EF8
