Page 7 <br /> <br /> <br />EXHIBIT A <br /> <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br /> <br />CITY OF PLEASANTON <br />AND <br />CityServe of the Tri-Valley <br /> <br />JULY 1, 2024 THROUGH JUNE 30, 2025 <br /> <br />Project No. 2024591 <br />At-Risk Family Stabilization and Homeless Prevention Program <br /> <br />The project to be undertaken by CityServe of the Tri-Valley shall involve: <br /> <br />The At-Risk Family Stabilization and Homeless Prevention Program is dedicated to assisting 300 families on the verge of <br />homelessness or facing housing instability. Emphasizing comprehensive care coordination, collaboration with schools, <br />partnerships with local providers and Tri-Valley cities, and drop-in hours at the Livermore office, the program offers immediate <br />intervention and support services. The program is non-clinical and inclusive services, delivered by staff and volunteers proficient <br />in multiple languages, ensure accessibility. Residents can access the program through referrals, a dedicated careline 925-222- <br />CARE, community outreach pop-ups, and drop-in hours. Securing funding is critical for preventing homelessness, ensuring <br />comprehensive care, and fostering resilient families. Without funding, the risk of family homelessness increases, access to <br />stabilization services diminishes, and community well-being faces potential strain. Funding is indispensable for sustaining this <br />vital initiative and promoting stable, supported families in our diverse community. <br /> <br />The project shall conform substantially to the scope of work described above. [However, the targets and service <br />goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City <br />of Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows: <br /> <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City’s HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission. <br />c) If the proposed change is not in conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br />Docusign Envelope ID: 4B9F600C-1E07-4246-A134-83C781E8A29F
