Page 7 <br />EXHIBIT A <br /> <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br /> <br />CITY OF PLEASANTON <br />AND <br />Tri-Valley Haven <br /> <br />JULY 1, 2024 THROUGH JUNE 30, 2025 <br /> <br />Project No. 2024618 <br />Homeless and Family Support Services (Sojourner + Food Pantry) <br /> <br />The project to be undertaken by Tri-Valley Haven shall involve: <br /> <br />Funding will support TVH’s Homeless and Family Services programs (HFS). TVH’s family shelter, Sojourner House, <br />provides homeless clients with support services while the Food Pantry (FP) provides nutritious food to low income & <br />homeless Tri-Valley residents. <br />Sojourner House will continue to provide homeless clients (women, women with children, two parent families with <br />children, men with children) with shelter and support services. Clients are offered up to six months shelter residency. <br />Shelter clients are provided case management, life skills classes and counseling services. <br />The Food Pantry will continue to provide groceries to Tri-Valley residents. Clients select from our daily inventory of <br />food and choose which items they prefer. In addition to groceries, clients can also receive social service referrals at <br />the FP. <br />The Food Pantry receives food through the Alameda County Community Food Bank monthly and participates in the <br />local Grocery Rescue program on weekday mornings. <br />Tri-Valley Haven will serve 400 Pleasanton residents. <br /> <br />The project shall conform substantially to the scope of work described above. [However, the targets and service <br />goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City <br />of Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows: <br /> <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City’s HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission. <br />c) If the proposed change is not in conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br /> <br /> <br /> Docusign Envelope ID: 9660A323-E305-4735-8869-A48A2C99B23E
