Page 7 <br />EXHIBIT A <br /> <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br /> <br />CITY OF PLEASANTON <br />AND <br />Sunflower Hill <br /> <br />JULY 1, 2024 THROUGH JUNE 30, 2025 <br /> <br />Project No. 2024603 <br />Sunflower Hill Program Support at Irby Ranch <br /> <br />The project to be undertaken by Sunflower Hill shall involve: <br /> <br /> <br /> <br />Funding will be used to support programs at Sunflower Hill at Irby Ranch in Pleasanton. <br />Completed in September 2020, Sunflower Hill at Irby Ranch is a residential community providing affordable, <br />independent housing for people with intellectual and developmental disabilities (I/DD) in a welcoming and <br />supportive community. Each apartment at Irby Ranch is its own private residence and is rented to income- <br />qualified residents at a rate of 20–60% AMI. Sunflower Hill at Irby Ranch is the first independent, affordable <br />residential community for people with I/DD in the Tri-Valley area. The program will serve approximately 36 <br />Pleasanton residents. <br />The project shall conform substantially to the scope of work described above. [However, the targets and service <br />goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City <br />of Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows: <br /> <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City’s HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission. <br />c) If the proposed change is not in conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br />Docusign Envelope ID: EEA84CD8-9F2A-452A-B60D-5EAA5D6E393D
