Docusign Envelope ID: F0E2385B-1 BCB-41 ED-B541 -2D5379A0908A EXHIBIT A CITY OF PLEASANTON HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM BETWEEN CITY OF PLEASANTON AND Centro Legal de la Raza JULY 1, 2024 THROUGH JUNE 30, 2025 Project No. 2024588 Tri-Valley Fair and Secure Housing Project The project to be undertaken by Centro Legal de la Raza shall involve : Our primary goal and objective is to enhance the quality of life for low-income tenants of Pleasanton via housing related legal services to prevent homelessness and to promote self-sufficiency, creating housing stabilization . To assist with those outcomes we have set the following performance goals : Performance Goal 1: 25* Tenants receive legal consultation for housing related problems such as resources and information about their legal options and rights. Performance Goal 2: 5* Tenants receive legal representation for housing related issues. *The goals above are just estimates , depending on client need the ratio of legal consultation to legal representation cases may be adjusted in order to be responsive to the community served, with the total goal remaining at 30 low- income tenants served. The project shall conform substantially to the scope of work described above . [However , the targets and service goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount requested and the amount approved .] Any request to modify the scope of the project shall be subm itted to the City of Pleasanton 's HHSG administrator in the form of a letter. Requests shall be processed as follows : a) If the proposed change is in substantial conformance with the original project scope as determined by the City 's HHSG program administrator, the change may be approved by City staff. b) If the proposed change is in partial conformance with the original project scope as determined by the City 's HHSG program administrator, the change will be submitted for review and approval by the City 's Human Services Commission and/or Housing Commission. c) If the proposed change is not in conformance with the original project scope as determined by the City's HHSG program administrator, the change will be submitted for review and approval by the City's Human Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a change to the Annual Action Plan.
