Docusign Envelope ID: CA40CEF5-FCF4-4B9C-9DED-480ECB315666 <br />EXHIBIT A <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br />CITY OF PLEASANTON <br />AND <br />Legal Assistance for Seniors <br />JULY 1, 2024 THROUGH JUNE 30, 2025 <br />Project No. 2024598 <br />Legal and Supportive Services, Medicare Counseling and Education for Pleasanton Seniors <br />The project to be undertaken by Legal Assistance for Seniors shall involve : <br />LAS will provide legal services to 7 seniors , education to 150 people via 7 educational events, and Medicare counseling to 50 <br />people . <br />LAS 's 8 attorneys, case manager, legal advocates , and legal volunteers will provide legal services. Areas of Jaw covered by <br />LAS include elder abuse, housing, conservatorship , guardianship , public benefits, health Jaw, limited conservatorship , and <br />naturalization . Sen iors contact the legal services program directly or through a referral . They may also be provided services by <br />our case manager. <br />LAS 's Health Insurance Counseling and Advocacy Program (HICAP) provides counseling on Medicare and related issues to <br />seniors and people with disabilities . Counseling is provided by 30 state-registered volunteer HICAP counselors . 10 paid staff <br />counselors provide supervision and support to the volunteers. HICAP appointments are available both in -person and virtually . <br />To make an appointment, clients contact LAS directly. <br />LAS 's Community Education Program gives a wide range or presentations and conducts extensive outreach to spread the <br />word about LAS 's services . All presentations are free and offered in multiple languages . They are typically accessed at the <br />senior center or another site where seniors congregate . <br />The project shall conform substantially to the scope of work described above. [However , the ta rgets and service <br />goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City <br />of Pleasanton 's HHSG administrator in the form of a letter. Requests shall be processed as follows: <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City 's HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the original project scope as determined by the City's <br />HHSG program administrator, the change will be submitted for review and approval by the City 's Human <br />Services Commission and/or Housing Commission . <br />c) If the proposed change is not in conformance with the original project scope as determined by the City's <br />HHSG program administrator, the change will be submitted for review and approval by the City 's Human <br />Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br />Page 7 <br />Docusign Envelope ID: 2D046EA3-3EAC-4B50-9E52-5CAAFF47F1ED
