Page 7 EXHIBIT A CITY OF PLEASANTON HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM BETWEEN CITY OF PLEASANTON AND CityServe of the Tri-Valley JULY 1, 2024 THROUGH JUNE 30, 2025 Project No. 2024592 Homeless Intervention & Crisis Stabilization Program The project to be undertaken by CityServe of the Tri-Valley shall involve: The Homeless Intervention and Crisis Stabilization Program in Pleasanton provides vital non-clinical assistance to individuals and families (in multiple languages) facing homelessness or housing crises. Targeting Pleasanton residents, the program aims to enhance well-being and housing stability. Population Served: Primarily serving 92 residents at risk of homelessness, including those facing eviction or sudden loss of housing (< 50% AMI) Access to Program: Residents access the program through referrals, our main Careline 925-222-CARE, online requests, in-person drop-ins (Livermore Public office), and community outreach pop-ups. Impact of Non-Funding: Without funding, there's a risk of increased homelessness, limited stabilization services, and compromised community well- being. Funding is crucial for immediate support and long-term community resilience. The project shall conform substantially to the scope of work described above. [However, the targets and service goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City of Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows: a) If the proposed change is in substantial conformance with the original project scope as determined by the City’s HHSG program administrator, the change may be approved by City staff. b) If the proposed change is in partial conformance with the original project scope as determined by the City’s HHSG program administrator, the change will be submitted for review and approval by the City’s Human Services Commission and/or Housing Commission. c) If the proposed change is not in conformance with the original project scope as determined by the City’s HHSG program administrator, the change will be submitted for review and approval by the City’s Human Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a change to the Annual Action Plan.
