Page 7 <br />EXHIBIT A <br /> <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br /> <br />CITY OF PLEASANTON <br />AND <br />Assistance League of Amador Valley <br /> <br />JULY 1, 2024 THROUGH JUNE 30, 2025 <br /> <br />Project No. 2024585 <br />Assistance League of Amador Valley <br />Operation School Bell <br /> <br />The project to be undertaken by Assistance League of Amador Valley shall involve: <br /> <br />Operation School Bell® is Assistance League’s signature program. The mission of Operation School Bell® programs <br />is to provide children in need with appropriate assistance to enhance self-esteem, promote learning, and regular <br />school attendance. Our programs under the Operation School Bell® umbrella are rooted in education and family. <br />Projects include supporting up to 400 kids in the valley with new school clothes, donating nearly 1,500 books to <br />children to take home as their own, and supporting the Gathering Place in Pleasanton by supplying reading books, <br />games, and gift cards for activities the kids can do together with their birth parents during these visits - among other <br />actions that support fosters and unhoused youth. <br /> <br />The project shall conform substantially to the scope of work described above. [However, the targets and service <br />goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City <br />of Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows: <br /> <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City’s HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission. <br />c) If the proposed change is not in conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br />Docusign Envelope ID: AF0A790E-72BA-4DEC-AEC7-74333D1245D7
