Docusign Envelope ID: 9533A8EA-4B7E-4202-B63F-13BB6034E8B2 EXHIBIT A CITY OF PLEASANTON HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM BETWEEN CITY OF PLEASANTON AND Abode Services JULY 1, 2024 THROUGH JUNE 30, 2025 Project No. 2024583 Tri-Valley Rapid Re-Housing Program The project to be undertaken by Abode Services shall involve: Rapid Re-Housing is an intervention designed to help individuals and families to quickly exit homelessness and return to permanent housing . The core components of a Rapid Re-Housing program are housing identification , move-in and rental assistance (including prevention/arrears payments), and Rapid Re-housing case management and services. The program offers participants 12 -18 months of rental assistance . The program manager can approve extensions beyond 12 months , based on demonstrated need. Every 90 days, the Housing Service Coordinator completes an income assessment and rental assistance typically decreases , as the goal is to have participants gradually move toward payment of the full lease amount. Prevention households and those receiving gap assistance typically receive subsidy for shorter periods of time (e .g., back rent and a few additional months of support) and lighter to very minimal services . The HSG will also provide service connections and coordination , to include referrals to employment search support; benefits; parenting support and childcare ; and a variety of other services . All program activities are overseen by the Program Manager, who supervises the Housing Service Coordinator, and by the Director of Housing, a member of the Abode Services Senior Management Team. In FY 2024125, Abode Services will serve six (6) Pleasanton households with these funds . These six (6) households will receive Rapid Re-Housing assistance and related services . The project shall conform substantially to the scope of work described above. [However, the targets and service goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City of Pleasanton 's HHSG administrator in the form of a letter. Requests shall be processed as follows: a) If the proposed change is in substantial conformance with the original project scope as determined by the City's HHSG program administrator, the change may be approved by City staff. b) If the proposed change is in partial conformance with the original project scope as determined by the City's HHSG program administrator, the change will be submitted for review and approval by the City 's Human Services Commission and/or Housing Commission. c) If the proposed change is not in conformance with the original project scope as determined by the City's HHSG program administrator, the change will be submitted for review and approval by the City's Human Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a change to the Annual Action Plan.
