Docusign Envelope ID : 6FB15333-3C4F-4E4D-9559-AA973F486F7C <br />EXHIBIT A <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br />CITY OF PLEASANTON <br />AND <br />Axis Community Health <br />JULY 1, 2025 THROUGH JUNE 30, 2025 <br />Project No. 2024586 <br />Enrollment Specialist <br />The project to be undertaken by Axis Community Health shall involve : <br />In 2024 , more Californians need help enrolling in healthcare for the first time and re-enrolling in public health insurance due to <br />the Medi-Cal redetermination process. There are two main challenges regarding healthcare insurance enrollment . Firstly, the <br />Medi-Cal redeterm ination process may lead to individuals losing their healthcare coverage, requiring them to re-enroll in eligible <br />insurance plans . Secondly, new rules perm it enrollment for individuals previously ineligible for Medi-Cal. However, the <br />enrollment process is intricate , posing difficulties for many to navigate independently. Our Enrollment Specialist can ass ist both <br />groups by determining their eligible insurance options and guiding them through the enrollment process for public health <br />insurance. During this grant period, Axis Community Health will provide services to at least 90 unduplicated Pleasanton clients. <br />The project shall conform substantially to the scope of work described above. [However, the targets and serv ice <br />goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City <br />of Pleasanton 's HHSG admin istrator in the form of a letter. Requests shall be processed as follows : <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City's HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the orig inal project scope as determined by the City's <br />HHSG program administrator, the change will be submitted for review and approval by the City 's Human <br />Services Commission and/or Housing Commission. <br />c) If the proposed change is not in conformance with the original project scope as determined by the City's <br />HHSG program administrator, the change will be submitted for review and approval by the City's Human <br />Services Commission and/or Housing Commiss ion and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br />Page 7 <br />Docusign Envelope ID: 561334F4-4433-43FE-A104-0E7BB8C651F2
