<br />EXHIBIT A FY 2024/25 – RATE SHEET <br />STREETS, TRAFFICE SAFETY & SIDEWALK <br />MAINTENANCE & REPAIRS <br />“SIDEWALK OFFSET REPAIR” <br /> <br />SCOPE OF SERVICES <br />On an As-Needed Basis, Contractor will provide the City with maintenance and repair services for City <br />sidewalks. <br /> <br />Contractor shall guarantee repair slope of meets current Americans with Disabilities Act minimum <br />slopes. <br /> <br />Supervision and Communication <br />A supervisor/foreman representing the contractor shall be readily available to meet with the Project <br />Manager or Designee as needed for the purposes of inspecting work, correcting problems and conflicts, <br />and coordinating work schedules. <br /> <br />The Contractor shall furnish all personnel, supplies, and materials needed for the completion of the <br />assigned project. Contractor is responsible for supplying all necessary equipment and tools to perform <br />the assigned work, whether owned or rented from a third party. Contractor will be responsible for <br />removing and disposing of any sidewalk debris removed from existing sidewalk during the course of <br />work performed. <br /> <br />Contractor’s crews should be identified either by a Company uniform, or by a safety vest worn outside <br />other clothing with the Company name clearly displayed. <br /> <br />All damages to private fencing, homes, backyard items, or ruts left by equipment shall be reported by <br />the Contractor to the City and will be repaired or replaced by the Contractor at no additional cost. <br /> <br />All OSHA Safety Requirements must be adhered to by the Contractor and/or any subcontractors while <br />on-site. <br />RATE SHEET <br />The cost quoted below shall include all taxes and all other charges, including travel expenses, and is the <br />cost the City will pay for the term of any contract that is a result of this bid. Special Note: When filling <br />out the specific rate sheet please insert “N/A” for components of work that can’t be provided. <br /> <br />Bidder hereby certifies to City that all representations, certifications, and statements made by Bidder, as <br />set forth in this Bid Form and attachments are true and correct and are made under penalty of perjury <br />pursuant to the laws of California. <br /> <br /> <br />Docusign Envelope ID: D5A391BD-DDF5-4DBF-90C7-138B00DFD5AD
