EXHIBIT B <br /> <br />As submitted from the Bid Proposal RFP OSD 2021-002 by Du-All, LLC <br /> <br /> 7.0 FEES <br /> <br />Du-All Safety believes a minimum of 24 hours per month is appropriate to support a City the <br />size of the City of Pleasanton, its Safety Officer(s), safety committee, and all their business <br />operation safety efforts. <br /> <br />a) Half-day rate of $1,200. <br /> <br />b) Full-day rate of $1,950. <br /> <br />c) The hourly rate for all other consulting work is $170.00 per hour. <br /> <br />d) Per-class/per-month recording & storage fee of $300 for all classes requested to be <br />recorded and made available 24/7 with unlimited employee access. <br /> <br />e) Any requested safety equipment purchases, including PPE, medical surveillance, DOT, <br />MUTCD manuals, 1st/CPR registration, air/soil/mold lab costs, will be invoiced at cost <br />plus 10%. <br /> <br />f) Any existing recordings will be charged at $200 a month and all new training videos <br />will be charged at $300 a month. <br /> <br />DocuSign Envelope ID: D4FD59B7-5718-4765-B842-AC6F6CB082C1
