EXHIBIT A <br />Scope of Consultant's Services <br />CityServe of the Tri-Valley <br />Scope of Work FY25 <br />CityServe of the Tri-Valley shall provide the following services: Caring Visiting, Care Coordination, Family <br />Caregiver Support, Mental Health Counseling, Wellness Education and Outreach, Exercise and Nutrition <br />Classes, Health Screening/Exams Information and Referral to the Pleasanton Senior residents to be <br />reimbursed up to $88,000 per City fiscal year for program expenditures. <br />Specific Services to be performed according to this Agreement for reimbursement are: <br />Care Coordination: <br />• On-site, drop-in case management and crisis intervention services available Monday-Friday, <br />9am-4pm at 5353 Sunol Blvd., Pleasanton, California. This would include having a qualified staff <br />person available to make immediate resolutions and/or referrals. <br />• Home visit assessments with follow-up care plans including recommendations to support senior <br />independence and viability. <br />Foot Care and Health events/class coordination: <br />• Provide regular footcare appointments at Pleasanton Senior Center; no less than 6 per year. <br />• Ability to host health screenings onsite. <br />• Connect patrons to various local health fairs and workshops that offer free health screenings. <br />Family Caregiver Support Services: <br />• Provider will visit and assess client's needs, and upon assessment, provide a list of pre-screened <br />workers for in-home supportive services. <br />Caring Visiting: <br />• Provide companionship through home visits and telephone calls. Agency will serve at least 100 <br />unduplicated seniors annually. Information and Referral - In Kind <br />Education/Outreach- In Kind: <br />• Develop and maintain a detailed referral services directory for low-income seniors, updated bi- <br />annually. <br />• Facilitate six (6) community outreach activities for seniors to educate them about available <br />resources in community. <br />• Provide education on Alcohol and Drug Abuse Prevention. <br />• Exercise Classes to supplement Pleasanton Senior Center fitness programs. <br />• Offer six (6) Nutrition classes per year focused on healthy eating habits. <br />Metrics <br />CityServe will provide documentation indicating reimbursable expenditures and Pleasanton Residents <br />served. <br /> <br />DocuSign Envelope ID: 9B715611-3FB6-44C3-AD13-DB1E241329ABDocuSign Envelope ID: 9F6B0B74-8CA4-424F-8E79-ADAD28CB16FF
