7 <br /> <br />EXHIBIT A – SCOPE OF SERVICES/WORK <br /> <br /> <br />GENERAL ITEMS: <br />x City Project Manager - Project Manager will be the Facilities Supervisor or his Designee <br /> <br />x Contractor Entry into OSC – Contractor will need to provide a list of all employee’s names. Once <br />approved, Contractor will be issued a temporary Proximity Card for on-site entry to project location. <br />Contractor will be responsible to gaining access to the OSC yard for his employees and subcontractors. At <br />the end of the Projector Contractor must return the Proximity Card back to the Project Manager. <br /> <br />x Hours of Services – will be 7am to 3:30pm; any variation in start/end times must be approved prior to <br />with the Project Manager and agreed upon by both parties <br /> <br />x Uniforms - Contractor’s crews should be identified either by a Company uniform, or by a safety vest worn <br />outside other clothing with the Company name clearly displayed <br /> <br />x Materials - Contractor shall furnish, as part of this agreement, all necessary personnel, supplies and <br />materials needed for the completion of the identified project. Contractor will be responsible for removing <br />and disposing of any construction waste. Debris and trash shall be removed at the end of each day’s work. <br />All defective materials shall be removed in accordance with all applicable rules, regulations, codes, law, <br />ordinances, statues, etc. <br /> <br />x Equipment and Tools – Contractor is responsible for supplying all necessary equipment and tools to <br />perform the project as outlined, whether owned or rented with a third-party agency <br /> <br />x Supervision - Contractor must provide for adequate supervision and inspection of all work performed to <br />ensure that each requirement of these specifications is consistently met <br /> <br />x Communication - A supervisor/foreman representing the contractor shall be readily available to meet with <br />the Project Manager or Designee as needed for the purposes of correcting problems, conflicts, and <br />coordinating work schedules <br /> <br />x Inspection - Inspection by Project Manager or Designee shall be made during normal City operating <br />hours, unless otherwise arranged <br /> <br />x Satisfaction - The progress and standard of quality of work to be accomplished shall be to the degree <br />reasonably acceptable to the Project Manager. In the event the Project Manager or his designee determines <br />contractor's work is unsatisfactory, contractor will be required to perform the additional work at no cost to <br />the City <br /> <br />x Damages - All damages to sprinklers and/or surrounding work areas shall be reported by Contractor to the <br />City and will be repaired by the Contractor at no additional cost <br /> <br />x Safety - All OSHA Safety Requirements must be adhered to by the Contractor and/or any subcontractors <br />while on-site <br /> <br />DocuSign Envelope ID: A09C14EB-53DE-46B6-BC81-69B61C978439
