Exhibit A <br /> <br />Statement of Work (SOW): Axis Community Health and City of Pleasanton <br />Revised for May to November 2024 <br />Introduction: <br />This Statement of Work (SOW) outlines the collaboration between Axis Community Health and the City <br />of Pleasanton to allocate one half-hour per week for a medical physician to provide healthcare services <br />to the sheltered/unsheltered unhoused community members in Pleasanton. The primary focus will be <br />on those individuals disproportionately impacted by substance use disorders. This partnership aims to <br />improve the health and well-being of the unhoused population by ensuring access to medical care. <br />Background: <br />The City of Pleasanton recognizes the need to address the healthcare needs of its unhoused residents. In <br />partnership with the Alternate Response Unit, Axis Community Health has agreed to collaborate with <br />the city to allocate medical resources and expertise. <br />Scope of Work: <br />Axis Community Health commits to providing the following services as part of this initiative: <br />Physician Team Availability: Axis Community Health will allocate a qualified medical physician team for <br />one half-hour each week to provide medical care to unhoused community members within the city of <br />Pleasanton. <br />Medical Services: During the allocated one half-hour, the physician team will offer medical consultation, <br />assessments, and treatments as deemed necessary. This may include but is not limited to: <br />-General health check-ups <br />-Diagnosis and treatment of common ailments <br />-Prescription and dispensing of medication, as appropriate <br />-Referral to specialists or ancillary services, if required <br />-Bridge team available as needed, including telepsychiatry. Both in person and telehealth options <br />available as follow up options <br />-Social drivers of health screenings and referrals. <br />-Provide connection to resources and health insurance enrollment, when needed <br />Care Coordination: Axis Community Health will provide up to one hour per week of care coordination <br />with the Alternate Response Unit clinician/representative to facilitate appointment access. The Care <br />Coordinator will work collaboratively with the physician team to assist with follow-up services, as <br />needed. <br />Data Reporting: Axis Community Health will maintain records of the services provided, including the <br />number of patients seen.
