EXHIBIT A <br />Pleasanton Homeless Intervention Program <br />Scope of Work <br />FY 2023/2024 <br />CityServe of the Tri-Valley on behalf of the City of Pleasanton will provide: <br />1) Housing relocation navigation and stabilization and short-and/or medium-term <br />(minimum of 6 months) rental assistance to help homeless individuals or family to move as <br />quickly as possible into permanent housing and achieve stability in that housing . <br />2) Supportive/case management services that include street outreach , mental health <br />services, emergency health services, employment assistance and job training, life skills <br />training, and substance abuse treatment services. <br />All PLHA funds will be used to support households with Extremely Low Income (30% of <br />AMI and below). The homeless intervention program provides outreach services to and <br />case management for literally homeless individuals for a pathway towards permanent <br />housing.
