If not, please explain how you plan to assess whether your clients are "eligible beneficiaries". <br />Our data is collected using the Homeless Management Information System (HMIS/Clarity via Alameda County) for the <br />unhoused population and WellSky SAMS Database Software For our senior clientele as well as our in-house custom <br />database for all clientele. <br /> <br />9. Provide the projected number of PLEASANTON CLIENTS who would be served by this program/project in fiscal <br />year (2024/2025). <br /> <br />10. Please list the total projected number of PLEASANTON CLIENTS your agency proposes to serve through this <br />program/project for each income category below in fiscal year 2024/2025. <br />(The total number of persons indicated in question #9 must match the total number listed in the income categories below.) <br /> <br />11. Provide a brief summary of the program/project, including the population served and how this population will <br />access this program/project, for which your agency is seeking funding. If you do not receiving funding, what would <br />be the impact? <br />Our Homeless Intervention and Crisis Stabilization Program in Pleasanton provides vital non-clinical assistance to individuals <br />and families (in multiple languages) facing homelessness or housing crises. Targeting Pleasanton residents, the program <br />aims to enhance well-being and housing stability. <br />Population Served: <br />Primarily serving residents at risk of homelessness, including those facing eviction or sudden loss of housing. (50 AMI and <br />below) <br />Access to Program: <br />Residents access the program through referrals, our main Careline 925-222-CARE, online requests, in-person drop-ins <br />(Livermore Public office), and community outreach pop-ups. <br />Impact of Non-Funding: <br />Without funding, there's a risk of increased homelessness, limited stabilization services, and compromised community well- <br />being. Funding is crucial for immediate support and long-term community resilience. <br /> <br />12. For this grant request, please list the agencies you will collaborate with, providing specifics on your <br />collaborative efforts. <br />We coordinate resource efforts with faith groups, non-profits, school districts, police departments, businesses, and <br />government agencies in order to create opportunities for intentional collaboration and regional impact. CityServe hosted two <br />faith-based and one service provider "Collab Lab" to share resources, network and discuss collaboration in 2023 with over 75 <br />providers in attendance. In 2024, we are hosting four service provider community meetings on Jan 23, April 23, July 23rd and <br />Oct 22nd in Pleasanton.
