The proposed use shall conform substantially to the written narrative, site plan, and floor plan, Exhibit "A", dated "Received March 13, 1997", on file with the Planning Department, except as modified by these conditions. 2. If operation of the proposed use results in conflicts pertaining to parking, noise, traffic, or other impacts, at the discretion of the Planning Director, this use permit may be submitted to the Planning Commission for subsequent review at a public hearing. If necessary, the Commission may modify or add conditions of approval to mitigate such impact, or may revoke the use permit. 3. If additional activities beyond that proposed in the written narrative, dated Received March 13, 1997", on file with the Planning Department are desired, prior City review and approval are required. Such modifications shall be submitted to the Planning Director, who shall refer the matter to the Planning Commission if determined to be substantial. 4. No more than two licensed massage therapists shall operate within the designated tenant suite. These conditions of approval shall apply to each therapist using the subject suite, whether they operate as separate or combined businesses. 5. The hours of operation for both massage therapists shall be limited to Monday through Saturday, 9 a.m. to 9 p.m.. Minor changes to the hours of operation may be submitted for review and approval by the Planning Director. 6. The applicant shall obtain all other applicable City permits prior to the commencement of the use. 7. This use permit is not transferable to another address. A new application must be made if a massage therapist relocates to another site. 8. No signage for the business is approved as part of this application. Any signage shall conform to the previously-approved sign program for the building and shall be subject to the review and approval of the Planning Director prior to installation. 9. No massage therapist may operate until the required Police Department approvals and registrations are obtained. Such registrations shall remain in effect as long as the business is in operation. {end}
