EXHIBIT <br />EXHIBIT A - SCOPE OF SERVICES <br />City Project Manager- Project Manager will be the Facilities Supervisor or his Designee <br />Contractor Entry into OSC —Contractor will need to provide a list of all employee's names. Once <br />approved, Contractor will be issued a temporary Proximity Card for on-site entry to project location. <br />Contractor will be responsible to gaining access to the OSC yard for his employees and <br />subcontractors. At the end of the Projector Contractor must return the Proximity Card back to the <br />Project Manager. <br />Hours of Services—will be lam to 3:30pm; any variation in start/end times must be approved prior <br />to with the Project Manager and agreed upon by both parties <br />Uniforms - Contractor's crews should be identified either by a Company uniform, or by a safety <br />vest worn outside other clothing with the Company name clearly displayed <br />Materials - Contractor shall furnish, as part of this agreement, all necessary personnel, supplies and <br />materials needed for the completion of the identified project. Contractor will be responsible for <br />removing and disposing of any construction waste. Debris and trash shall be removed at the end of <br />each day's work. All defective materials shall be removed in accordance with all applicable rules, <br />regulations, codes, law, ordinances, statues, etc. <br />Equipment and Tools —Contractor is responsible for supplying all necessary equipment and tools <br />to perform the project as outlined, whether owned or rented with a third-party agency <br />Supervision - Contractor must provide for adequate supervision and inspection of all work <br />performed to ensure that each requirement of these specifications is consistently met <br />Communication -A supervisor/foreman representing the contractor shall be readily available to <br />meet with the Project Manager or Designee as needed for the purposes of correcting problems, <br />conflicts, and coordinating work schedules <br />Inspection - Inspection by Project Manager or Designee shall be made during normal City operating <br />hours, unless otherwise arranged <br />Satisfaction - The progress and standard of quality of work to be accomplished shall be to the <br />degree reasonably acceptable to the Project Manager. In the event the Project Manager or his <br />designee determines contractor's work is unsatisfactory, contractor will be required to perform the <br />additional work at no cost to the City <br />Damages -All damages to sprinklers and/or surrounding work areas shall be reported by Contractor <br />to the City and will be repaired by the Contractor at no additional cost <br />Safety - All OSHA Safety Requirements must be adhered to by the Contractor and/or any <br />subcontractors while on-site <br />DocuSign Envelope ID: C872947D-41CE-4639-8F1A-296D6CD20936
