Describe the project/program funded with this grant, the current status of the project/program, and any <br />significant actions taken during the reporting period. <br />Report 1 <br />The current status of our program is as follows: We have served 31 new homeless individuals (54 total in household) <br />during this time period and we have a total of 56 homeless clients (80 total in household) in our current caseloads and/or <br />on our radar overall for Pleasanton that we are still serving from the last reporting period. Several clients are getting very <br />close to housing. As of today we have 11 individuals in hotels for winter relief. Here are some additional actions taken <br />during this reporting period. <br />Fielded over 80 crisis calls from unsheltered individuals <br />Had an average of 15 homeless individuals walk-in during drop-in hours (per day) at our Livermore office since November. <br />We currently do not have an office anymore in Pleasanton for homeless individuals therefore access to services has been <br />limited <br />We continue to provide care coordination sessions for clients facing homelessness by assisting with referrals to <br />specialized providers and work through action steps to produce stabilization. Many of these referrals were for mental <br />health, physical health and addiction support or detox. <br />We had a Pleasanton unsheltered family of six in our hotel program and they were just housed and are well on their way to <br />sustainability for the long term. Unfortunately, they could not afford housing in Pleasanton because they were on a <br />voucher so they were placed in Livermore. This has happened to another unsheltered senior client as well. <br />Report 2 <br />The current status of our program is as follows: We have served 51 new homeless individuals (72 total in household <br />during this time period and we have a total of 82 homeless clients(126 in household) in our current program or on our <br />radar in Pleasanton. <br />As clients go on their journey they move closer to having options for housing and other programs that will build stability. <br />Assisted clients with becoming document ready for housing <br />Advocated at the county level for better addiction and mental health services for many of the clients dealing with severe <br />issues that were creating obstacles holding them back from moving forward. <br />Collaborated with many organizations to ensure that a client was tapped into all services related to basic needs, <br />emotional health and well being, physical health, vocational and educational development, community connections and <br />financial stability. <br />Did Ride Alongs 2 times per month with Pleasanton homeless liaison officers to ensure that our most vulnerable residents <br />were connected to services. <br />Partnered with housing organizations like HACA, Goodness Village, Livermore Housing Authority, Eden Housing, MidPen <br />Housing and Saha Homes to assist clients in making connections to wait lists that were open. <br />Provided hotel shelter during severe cold winter nights between January and March to 8 households(18 individuals). <br />Collaborated with Pleasanton Unified School District to provide support to families and individuals who were unhoused.
