EXHIBIT A <br /> SCOPE OF WORK <br /> A. Account Manager/Support Staff <br /> Contractor shall provide a dedicated competent account manager who shall be responsible <br /> for the City account/contract. The account manager shall receive all orders from the City <br /> and shall be the primary contact for all issues regarding Bidder's response to this RFP and <br /> any contract which may arise pursuant to this RFP. <br /> The Contractor shall also provide adequate, competent support staff that shall be able to <br /> service the City during normal working hours, Monday through Friday, and during off-hour <br /> emergency operations. Such representative(s)shall be knowledgeable about the contract, <br /> services/products offered and able to identify and resolve quickly any issues including but <br /> not limited to order and invoicing problems. <br /> B. Contractor as Transporter and Arranger for Disposal of Wastes <br /> The Contractor will be expressly designated by the Parties hereto as the transporter of, and <br /> the arranger for the disposal of all wastes to be transported,managed, and disposed of under <br /> this Agreement. <br /> Upon the Contractor's acceptance of wastes identified for removal,transportation <br /> management and disposal under this Agreement,the Contractor,to the extent of its <br /> negligent acts or omissions, shall undertake risk of]oss of such wastes unless the waste is <br /> determined by the Contactor to be non-conforming in accordance with Section 7 procedures <br /> for non-conforming wastes. Contractor shall not be considered the generator or owner of the <br /> waste material which is transported, managed, and disposed of under this Agreement. <br /> C. Documentation,Certification and Record Requirements <br /> Contractor will provide all appropriate documentation, certifications and records for the <br /> transportation, management, and disposal of hazardous wastes in conformance with all <br /> applicable federal, state, and local laws and regulations, including, but not limited to: <br /> • bills of lading or non-hazardous waste manifests <br /> • hazardous waste manifests and continuation sheets <br /> • lab-pack inventory sheets <br /> • land disposal restriction notifications <br /> • hazardous waste profiles <br /> • certificates of destruction, decontamination, disposal and/or recycling <br /> Appropriate copies of the documentation listed above as well as any other documents <br /> required by the City's Environmental Services Division(ESD) will be furnished to the City's <br /> Program Manager or designee at the time of waste shipment. <br /> Closed originals of all uniform hazardous(and non-hazardous)waste manifests signed by a <br /> duly authorized representative of the receiving Treatment/Storage/Disposal Facility (TSDF) <br />
