§302(c)(4)Plan Rev 2/16122 <br /> 302(c 4 A)Describe the manner in which allocated funds will be used for eligible activities. <br /> The City of Pleasanton will allocate 50%of the PLHA funds to augment the following homeless services currently provided by CityServe of the Tri-Valley on behalf of the City.1)Housing relocation an <br /> stabilization and short-and/or medium-tens(minimum of 6 months)rental assistance to help homeless individual or family to move as quickly as possible into permanent housing and achieve stability i <br /> that housing,and 2)Supportive/case management services that include street outreach,mental health services,emergency health services,employment assistance and job training,life skills training, <br /> and substance abuse treatment services.The City will allocate the other 50%of the PLHA allocation to assist Tri-Valley Haven with operating costs,including rental costs and utilities,for providing 14 <br /> emergency shelter beds at a converted home to homeless survivors of domestic violence and their children after Tri-Valley Haven had to close one of its domestic violence shelter buildings due to mol <br /> and structural damage. Tri-Valley Haven is providing the temporary 14 emergency shelter beds while the new domestic violence shelter building is being funded and constructed. <br /> §302(c)(4XS)Provide a description of the way the Local government will prioritize investments that increase the supply of housing for households with incomes at or below 60 percent of Area Median <br /> Income(AMI). <br /> All PLHA funds will be used to support households with Extremely Low Income(30%of AMI and below). The homeless intervention program provide outreach services to and case management for <br /> literally homeless indviduals for a pathway towards permanent housing.
