Dates, times, addresses, and names of Commercial Customers that received technical 3059 <br />assistance/audits; and, 3060 <br />3. Report the percentage of total Commercial service volume that is Recyclables volume and 3061 <br />Organics volume (e.g., the percentage of Commercial service volume that is Recycling volume 3062 <br />equals the number of cubic yards of Commercial Recycling service per week divided by the sum 3063 <br />of the weekly Commercial Solid Waste, Recyclable Materials, and Organic Materials cubic yards 3064 <br />service per week). 3065 <br />I. Multi-Family Diversion and Technical Assistance Report 3066 <br />1. Summarize the technical assistance provided to Customers by identifying the number of site 3067 <br />visits conducted each month in the most-recently completed quarter, and submitting copies of 3068 <br />the Multi-Family Diversion auditing forms as required in Section 5.17.5; 3069 <br />2. Dates, times, addresses, and names of Multi-Family Customers that received technical 3070 <br />assistance/audits; and, 3071 <br />3. Report the average volume of Recyclables Collected per tenant unit per week (e.g., the number 3072 <br />of cubic yards of Multi-Family Recycling service per week divided by the total number of Multi-3073 <br />Family units in the City). 3074 <br />J. Pilot and New Programs. For each pilot and/or new program, provide activity related and narrative 3075 <br />reports on goals, milestones, and accomplishments. Describe problems encountered, actions taken 3076 <br />and any recommendations to facilitate progress. Describe vehicles, personnel, and equipment 3077 <br />utilized for each program. 3078 <br />K. Summary Assessment. Provide a summary assessment of the overall Solid Waste, Recyclable 3079 <br />Materials, and Organic Materials program from Contractor's perspective relative to the financial 3080 <br />and physical status of the program. The physical status assessment shall reflect how well the 3081 <br />program is operating in terms of efficiency, economy, and effectiveness in meeting all the goals and 3082 <br />objectives of this Agreement, particularly the Contractor’s Diversion goals. Provide 3083 <br />recommendations and plans to improve. Highlight significant accomplishments and problems. 3084 <br />L. Customer Accounts. Provide a detailed report of all Customers served in the City, indicating the 3085 <br />Service Level by material type of each. Such report shall include a summary of the number of 3086 <br />Customers at each Service Level. Include a list of Customers with invoices that are sixty (60) days 3087 <br />past due including the following information for each delinquent account: name, service address, 3088 <br />contact information, number of days the account is delinquent, method(s) the Contactor has used 3089 <br />to attempt collection of the bad debt including date of such attempt(s), and identification, if, and 3090 <br />when the Contractor reduced the Customer’s level of Collection services as a result of the account 3091 <br />delinquency. 3092 <br />M. Vehicle Inventory. Provide a report of all vehicles used for services under this Agreement, 3093 <br />identifying vehicle type (e.g. rear-load, automated side-load, roll-off, flat-bed, pick-up, etc.), make, 3094 <br />model, and year. 3095
