These entities and services are described in Table 3 <br /> below: <br /> Table 3: Tri-Valley Mental Health Resources <br /> Provider Location Services <br /> Alameda County Countywide System-wide point of contact for information, <br /> Behavioral Health: screening, and referrals for mental health services <br /> ACCESS Service for residents who have or are eligible for Medi-Cal. <br /> Telephone service staffed Monday-Friday with <br /> licensed clinicians and support. <br /> Alameda County Countywide Crisis intervention, assessment, and referrals <br /> Mobile Crisis Team (Limited) dispatched via telephone at 510-891-5600. <br /> Crisis Support Services Countywide Crisis intervention counselors available 24 hours a <br /> Suicide Prevention of day via 1-800-309-2131, support groups, and <br /> Alameda County counseling services. <br /> Alameda County Livermore Free education, advocacy, and support to family <br /> Family Education and caregivers of individuals with mental health issues. <br /> Resource Center <br /> (FERC) <br /> Bay Area Community Pleasanton Wellness education and programming, including <br /> Services (BACS); psychiatric support, for individuals with housing <br /> Valley Wellness insecurity and mental health challenges. <br /> Nami Tri-Valley Pleasanton NAMI Tri-Valley, in collaboration with other <br /> community agencies and organizations, provide <br /> support, resource information, education programs, <br /> and advocacy. <br /> *Due to COVID-19, walk-ins are not allowed, and services are provided via telephone. <br /> The programs and services in Tables 2 and 3 are funded through a variety of means, <br /> including but not limited to: <br /> • Proposition 63 funding (i.e. Mental Health Services Act) administered by <br /> Alameda County Behavioral Health. <br /> • Insurance programs and Medi-Cal. <br /> • Contributions or grants from other governmental agencies, foundations and/or <br /> individuals. <br /> The City of Pleasanton is currently providing funding for $70,000 under a two-year <br /> service agreement to Axis Community Health to provide behavioral health services for <br /> Pleasanton youth, specifically two age groups, 12-15 and 15-18. Ongoing group <br /> counseling programs provide expedited access to other medical and support services <br /> as appropriate to meet the needs of each participant. Through the Community Grants <br /> Program, the City provides funding to Nami Tri-Valley for $3,900 for education and <br /> support programs. <br /> Page 3 of 5 <br />
