Exhibit D <br /> PARATRANSIT VEHICLES <br /> This exhibit is intended to share a description of CITY vehicle maintenance standards and are to <br /> be used as a reference. <br /> Vehicle Use <br /> CONTRACTOR is leasing CITY vehicles 1305A, 1313 and 1314A for the term of this <br /> agreement. CONTRACTOR is responsible for storing, fueling, and maintaining leased vehicles <br /> during the lease term. <br /> Record Requirements <br /> Records of inspection shall be retained by the CONTRACTOR for one year and shall be made <br /> available for inspection upon request by any authorized employee of the CITY. <br /> Daily Vehicle Maintenance Record Requirement <br /> It is recommended the CONTRACTOR address vehicle maintenance and record requirements <br /> pursuant to California Code of Regulations, Title 13 (Motor Vehicles), Division 2, <br /> Chapter 6.5, Section 1215. <br /> Preventative Maintenace <br /> It is recommended that preventative maintenance happen within 10% of a scheduled interval. <br /> Scheduled interval is defined as 3,000 miles or 90 days, whichever comes first. Vehicles must be <br /> serviced within 300 miles on either end of the 3,000 mile interval. <br /> Vehicle Accident and Damage Reporting <br /> If a leased CITY vehicle is involved in an accident, the CONTRACTOR shall complete the <br /> CITY accident report form and forward to the Operations Services Department Operations <br /> Superintendent within two working days. <br /> Accidents involving injury must be reported to the CITY's Contract Manager within one <br /> working day. <br /> Vehicle and Equipment Failure <br /> In the event of a leased major vehicle failure, such as a complete engine or transmission failure, <br /> where the cost to repair is valued at more than the vehicle, the CONTRACTOR and CITY will <br /> meet and confer to address vehicle replacement. CONTRACTOR will notify the CITY fleet staff <br /> and will be responsible for having the vehicle transported to the Operations Service Center for <br /> assessment. <br />
