Exhibit D PARATRANSIT VEHICLES This exhibit is intended to share a description of CITY vehicle maintenance standards and are to be used as a reference. Vehicle Use CONTRACTOR is leasing CITY vehicles 1305A, 1313 and 1314A for the term of this agreement. CONTRACTOR is responsible for storing, fueling, and maintaining leased vehicles during the lease term. Record Requirements Records of inspection shall be retained by the CONTRACTOR for one year and shall be made available for inspection upon request by any authorized employee of the CITY. Daily Vehicle Maintenance Record Requirement It is recommended the CONTRACTOR address vehicle maintenance and record requirements pursuant to California Code of Regulations, Title 13 (Motor Vehicles), Division 2, Chapter 6.5, Section 1215. Preventative Maintenace It is recommended that preventative maintenance happen within 10% of a scheduled interval. Scheduled interval is defined as 3,000 miles or 90 days, whichever comes first. Vehicles must be serviced within 300 miles on either end of the 3,000 mile interval. Vehicle Accident and Damage Reporting If a leased CITY vehicle is involved in an accident, the CONTRACTOR shall complete the CITY accident report form and forward to the Operations Services Department Operations Superintendent within two working days. Accidents involving injury must be reported to the CITY's Contract Manager within one working day. Vehicle and Equipment Failure In the event of a leased major vehicle failure, such as a complete engine or transmission failure, where the cost to repair is valued at more than the vehicle, the CONTRACTOR and CITY will meet and confer to address vehicle replacement. CONTRACTOR will notify the CITY fleet staff and will be responsible for having the vehicle transported to the Operations Service Center for assessment.
