EXHIBIT A <br /> Scope of Services <br /> Senior Support Program of the Tri-Valley <br /> Senior Support Program of the Tri-Valley (SSPTV) shall provide the following services: <br /> Friendly Visiting, Case Management, In-home Caregiver Registry, In-home counseling, <br /> Family Caregiver Support, Mental Health Counseling, Wellness Education and <br /> Outreach, Exercise and Nutrition Classes, Senior Transport Program, Stepping out and <br /> Reconnecting, Health Screening/ Exams, and Information and Referral services to the <br /> Pleasanton Senior residents. Program expenditures shall be reimbursed by the City, up <br /> to $88,000. <br /> Specific Services to be performed according to this Agreement for reimbursement are: <br /> Case Management Services: <br /> • Provide home visit assessments with follow-up care plans including <br /> recommendations to support senior independence and viability. <br /> • Provide on-site, drop-in case management and crisis intervention services <br /> Monday-Friday, 9am-4:30pm at 5353 Sunol Blvd., Pleasanton, California (as <br /> allowed by Alameda County Health orders). This will include having a qualified <br /> staff person available to make immediate resolutions and/or referrals. <br /> Foot Care and Health Screenings: <br /> • Provide weekly footcare appointments at the Pleasanton Senior Center. <br /> • Work with City Staff to determine expansion of the program to accommodate a <br /> minimum of 40 residents, with a goal of eliminating the waiting list for Pleasanton <br /> Residents. <br /> • Conduct bi-monthly health screenings including, but not limited to: diabetes, <br /> blood pressure, hemoccult, and urine testing. <br /> In Home Registry and Family Caregiver Support Services: <br /> • Visit and assess client's needs, and upon assessment, provide a list of pre- <br /> screened workers for in-home supportive services. Will require a minimum of 15 <br /> hours per week. <br /> Friendly Visiting: <br /> • Provide companionship through home visits and telephone calls. Agency will <br /> serve at least 100 unduplicated seniors annually. Information and Referral - In <br /> Kind. <br /> Education/Outreach- In Kind: <br /> • Develop and maintain a detailed referral services directory for low income <br /> seniors, updated bi-annually. <br /> • Facilitate six community outreach activities for seniors to educate them about <br /> available resources in community. <br /> 4 <br />
