MATTERS BEFORE THE COMMISSION <br />4. Review of Community Development Block Grant (CDBG) Consolidated Annual <br />. Performance and Evaluation Reports (CAPER) for Fiscal Year 2018-2019 <br />Mr. Hernandez provided information about the Community Development Block Grant (CDBG) <br />Program requirements as they relate to the U.S. Department of Housing and Community <br />Development (HUD). He advised that the City is required to hold a public hearing to review <br />fiscal year 2018/2019 CDBG funded program accomplishments and then submit_a <br />Consolidated Annual. Performance and.Evaluation Report (CAPER) that will summarize CDBG <br />and other housing and human services projects for low_ and moderate income persons <br />supported by the City during FY 20128/2019. <br />Commissioners were advised by Mr. Hernandez that the Human Services. Commission should <br />now hold a public hearing to hear reports from the agencies that had received funding. <br />Chairperson Parikh opened the meeting for agency presentations at 7:13 p.m. <br />Axis Community Health – Phase 2 of Capital Project for Dental Services, Valerie Jonas <br />thanked the City of Pleasanton for providing funding for the dental project. She advised that a <br />Needs Assessment was conducted to help determine exactly what was needed. An existing <br />dental facility was found in a suitable Dublin location that already had five chairs and space for <br />being able add a sixth. Axis was able to enter into a long-term lease arrangement on. the <br />property by using CDBG funds and hire the appropriate staff members. Commissioners were <br />advised that the dental facility opened on February 5, 2019 and is providing all required dental <br />services. Dental services have been integrated Behavioral Health. <br />Commissioner Clare was provided information about the number of clients that have received <br />services since the facility opened in February. <br />Commissioner Sedlak discussed with Ms. Jonas the total funding that had been allocated to <br />Axis for this project and the amount that has spent. He also discussed the unused funds with <br />Mr. Hernandez who advised that Axis has five -years to use funds for CAP improvements. <br />Commissioner- Powers questioned whether any of the hired employees were dental students. <br />Ms. Jonas advised that all employees come from the clinic world and are aware of the <br />population type they service. She also advised Commissioner Gohil that no volunteers are <br />used because a high rate of productivity.is required. Hours of operation for the dental facility <br />was provided for Commissioner. Rubino-Brumm. <br />Tri -Valley Haven – Capital Improvements Project, Food Pantry, Shiloh Domestic Violence <br />Shelter and Services, Sojourner House Homeless Shelter and Counseling & Temp Restraining <br />Order Clinic -Commissioners were.provided an update on the Capital project and information <br />aboutthe project manager and architect that has been hired and the construction testing that <br />has been conducted. They were advised that it is. hoped the project will be sent out to bid for a <br />Human Services Commission <br />September 4, 2019 <br />Page 2 . <br />
