e. proof that the person has commercial general liability insurance in an
<br />amount not less than $2,000,000 per occurrence.
<br />6.24.050 APPLICATION FOR CITY REGISTRATION PERMIT
<br />A. Application. Any Person, except as herein otherwise provided, desiring to operate a
<br />Massage Business in the City of Pleasanton, shall first file with the Police Department an
<br />application for a City Registration Permit on forms provided by the Police Department. The
<br />application shall include all of the following:
<br />1. Applicant's legal name, including all names used in the present and past,
<br />residence and business addresses and telephone numbers, and email address. If the Applicant
<br />is a Massage Business, also include the Massage Business's legal name, proposed business
<br />address and telephone number, and optional email address. Applicants shall include the
<br />Applicant's residence address to allow for correspondence via United States mail;
<br />2. copy of a current and valid driver's license or identification issued by a state or
<br />federal governmental agency or other photographic identification bearing a bona fide seal by a
<br />foreign government;
<br />3. two recent, identical, passport -size, color photographs of the Applicant. The Police
<br />Chief shall have the right to take additional photographs of the applicant;
<br />4. legal names, including all names used in the present and past, of all Massage
<br />Business Owners;
<br />5. residence and business addresses and telephone numbers of all Massage
<br />Business Owners;
<br />6. a list of all of the Massage Business's Employees and Independent Contractors
<br />and copies of each of their CAMTC certifications or Massage Technician Permits;
<br />7. the form of business under which the Massage Business will be operating (e.g.,
<br />Sole Proprietorship, corporation, general or limited partnership, limited liability company, or other
<br />form);
<br />8. the name and street address of the owners and lessors of the real property on
<br />which the Massage Establishment is proposed to be conducted. If the Applicant is not the legal
<br />owner of the real property, the application shall be accompanied by a copy of the lease and an
<br />acknowledgment from the owners of the real property that the Massage Establishment is
<br />proposed to be conducted on the property;
<br />9. the Applicant's criminal convictions, if any, and whether or not the conviction has
<br />been expunged, or entered a plea of no contest, for all offenses other than traffic violations within
<br />10 years before the date of the application;
<br />10. any criminal charges, other than traffic violations, pending against the Applicant at
<br />the time of the application, the name and location of the court in which the charges are pending,
<br />and the applicable case numbers;
<br />11. as to any Applicant who has ever applied for a professional or vocational license,
<br />permit, or certificate related to Massage from any agency, board, city, county, city and county,
<br />territory, state, or other governmental jurisdiction and such jurisdiction denied the application, the
<br />date of such application for such license, permit, or certificate, the name of the jurisdiction that
<br />denied such license, permit, or certificate and the reasons for such denial;
<br />12. as to any Applicant who has ever held a professional or vocational license, permit,
<br />or certificate related to Massage issued by any agency, board, city, county, city and county,
<br />territory, state, or other governmental jurisdiction, the date of issuance of such license, permit, or
<br />certificate, whether such license, permit, or certificate is currently in effect and, if not, whether it
<br />has been revoked, suspended, restricted, or lapsed, and, if such license, permit, or certificate has
<br />been revoked, suspended, or restricted, the reasons therefore, and the names) of the governing
<br />jurisdiction;
