C. Display of Permit. <br /> 1. During a Massage Practitioner's working hours and at all times when the <br /> Massage Practitioner is inside a Massage Business or providing Offsite Massage, the Massage <br /> Practitioner shall wear his/her CAMTC certification or Massage Technician Permit such that it is <br /> prominently displayed and clearly visible. All other Employees shall wear a name badge that <br /> identifies the Employee's name and job title for the Massage Business. <br /> 2. When providing an Offsite Massage, each Massage Practitioner shall display to <br /> each Client the permittee's Offsite Permit. <br /> 3. When providing Massage at a Special Event, each Massage Practitioner shall <br /> display the person's CAMTC certification or Permit issued under this Chapter in an open and <br /> conspicuous place. <br /> 4. A copy of the Massage Business's City Registration Permit and each Massage <br /> Practitioner's CAMTC certification or Massage Technician Permit shall be displayed in an open <br /> and conspicuous place in the Reception and Waiting Area or similar open public place on the <br /> premises of the Massage Business. <br /> 5. A copy of the CAMTC certification, City Registration Permit, or of the Massage <br /> Technician Permit of each Massage Practitioner employed in or hired by the Business shall be <br /> displayed in the Reception and Waiting Area or similar open public place on the premises. <br /> CAMTC certification and Massage Technician Permits of former Massage Practitioners shall be <br /> removed as soon as those Massage Practitioners are no longer employed by or offering <br /> services through the Massage Business. <br /> D. Record of Services. For each Massage provided, the Massage Business that provided <br /> the Massage shall keep a complete record in English, in paper or electronic form, of the <br /> following information: the date and hour the service was provided; the service provided; the <br /> name of the Employee or Independent Contractor entering the information; the name of the <br /> Client; the name of the Massage Practitioner administering the Massage to the Client; the <br /> amount paid for the service, including tip, and the name of the administrator in charge of the <br /> Massage Business as described below in subsection 6.24.100(F). The records shall be <br /> immediately available for inspection and copying by police officers or other City officials charged <br /> with enforcement of this Chapter. These records may not be used by any Massage Practitioner, <br /> Massage Business Owner, or anyone else for any purpose other than as records of service <br /> provided and may not be provided to other parties by the Massage Practitioner, the Massage <br /> Business Owner, or any other Person unless required by law. The records shall be retained on <br /> the premises of the Massage Business for a period of three years. <br /> E. Responsible Person. The Massage Business Owner shall designate or employ a <br /> Massage Business administrator to be in charge of the Massage Business during all times the <br /> Massage Business Owner is not on site of the Massage Business. The name of the person <br /> currently in charge of the Massage Business shall appear in the record of service described in <br /> subsection 6.24.100(D). Either the Massage Business Owner or the Massage Business <br /> administrator shall be on site at the Massage Business at all times during business hours that: <br /> 1. Any Massage Business Employee or Independent Contractor, other than a single <br /> receptionist, is present at the Massage Business; or <br /> 2. Any member of the public is in any room of the Massage Business other than the <br /> Reception and Waiting area. <br /> 10 <br />
