8/7/2019 <br />Housing and Independent Living Services <br />22. For CAPER: Describe the accomplishments of the project or program funded through HHSG. Provide <br />detail on how the program responded to needs within the community. Describe any new and creative <br />methods the agency implemented to meet community needs <br />Report 1 <br />n/a <br />Report 2 <br />10 or 50% of the 20 Pleasanton consumers who were provided with 1:1 assistance chose Housing as their primary <br />or secondary goal. Finding affordable and, often accessible, housing frequently takes more than one year. So all of <br />these 10 housing consumers are still trying to achieve their long-range goal of moving into permanent, low-cost <br />housing. CRIL service coordinators continue to work with the consumers who have ongoing goals that have yet to <br />be met. <br />CRIL participated in several outreach events and facilitated workshops to educate Pleasanton residents with <br />disabilities about benefits, employment opportunities, healthy living options, assistive technology, travel training and <br />more. CRIL has increased its collaboration with Pleasanton Paratransit. CRIL continues to increase Pleasanton <br />consumers access to Assistive Technology - especially through the Disability Lending Library. <br />23. For CAPER: Does the agency feel this project or program was a success? How do you measure the <br />success? Did it meet or exceed the goals and outcomes described in the performance measures in the <br />original application? If not, why? <br />Report 1 <br />n/a <br />Report 2 <br />20 Pleasanton residents with disabilities were provided direct housing assistance or independent living services <br />during the 2018-19 year. Independent Living Services included: Benefits counseling, Advocacy, Assistive Technology <br />services and AT device loans, IL Skills training, Peer counseling, Personal Assistance Services, Employment <br />services, ADA assistance, and Information & Referral. <br />Another 280 Pleasanton residents with disabilities, for whom we do not have demographic details, and 35 <br />businesses were provided disability education, advocacy, technical assistance and information & referral and 1:1 <br />counseling time. However, we were only able to obtain demographic information on the 20 Pleasanton residents <br />directly served. <br />24. For CAPER: Describe any problems or delays encountered with the project or program. How were they <br />handled? What effects, if any, were there on costs?
