7/23/2019 <br />Tri -Valley Rapid Re -Housing Program <br />Report 1 <br />N/A <br />Report 2 <br />The TriValley Rapid Rehousing Program provides homeless families in the Pleasanton area with housing placement and a gradually decreasing <br />rental subsidy to help families stabilize and become self sufficient. We also provide case management to work on accomplishing housing <br />stability goals developed together with the family. Households will work to increase their income so they can afford their full rent once they exit <br />the program. The program will help with some homeless prevention which includes back rent and motels to keep families from becoming <br />homeless. <br />22. For CAPER: Describe the accomplishments of the project or program funded through HHSG. Provide detail on how the program <br />responded to needs within the community. Describe any new and creative methods the agency implemented to meet community <br />needs <br />Report 1 <br />N/A <br />Report 2 <br />The program successfully enrolled seven households in this Fiscal Year, we have placed four into housing while one remains in housing search. <br />We had two exits that resulted in participants remaining housed at exit. We had an additional four exits of participants who either moved out of <br />the Bay area or we were not able to maintain contact with. <br />Success stories: <br />1. We were able to keep a family housed in Pleasanton by negotiating with the landlord in the middle of her eviction. We were able to bring the <br />family current on back rent owed and the participant was exited from the program in good standing with her landlord. <br />2. We were able to house an individual who was homeless for over five years, who has an incurable disease and needed housing quickly. Within <br />five months he received a section 8 voucher and was able to remain housed in his same unit with a subsidy that he can afford on his own. <br />23. Did it meet or <br />exceed the goals and outcomes described in the performance measures in the original application? Describe any changes that made the project or program successful now or in the future. <br />Report 1 <br />N/A <br />Report 2 <br />Every effort was made to house these families in Pleasanton, but since self sufficiency after the program is a high priority, some households <br />also looked outside of Pleasanton to have a more affordable unit, or to be closer to work or family support. <br />25.
